Firstly lodge an FIR at the nearest police station along with a copy of the passport.
If you are unable to do the first step, book an appointment with the nearest Regional Passport Office (RPO).
During this, you have all the necessary documents suggested by the employee including the necessary documents like address proof, Aadhaar card, and birth certificate.
Applicants can also book the application process online through the official Passport Seva website.
Log in with your registered ID or you can also choose the 'Tatkal' option to get an early appointment.
Now choose the date of payment and appointment at your convenience.
Note that a processing fee will be required for lost passports.
