A health scheme is run by the Central Government, whose name is Ayushman Bharat Scheme. However, now many state governments have also joined this scheme, due to which the name of this scheme has been changed to 'Ayushman Bharat Pradhan Mantri Jan Arogya Yojana-Mukhyamantri Yojana'. Under this scheme, an Ayushman card of eligible people is made and after this, you can get free treatment up to Rs 5 lakh in the listed hospitals. But keep in mind that you need some documents at the time of application and if they are not there, your application may get canceled. So let us know what these documents are. These are the documents which are required
When you apply for an Ayushman card, you need some documents. These require the applicant's Aadhar card, ration card, residence certificate, and active mobile number.

Then you can apply:-
Step 1

If you are eligible for the scheme and you have the documents mentioned above, then you can apply.
In such a situation, first of all, you have to go to your nearest public service center.

Step 2
You have to first go to the public service center and meet the concerned officer.
These officers do the work of applying
After this, you will be asked for your documents

Step 3
Then you have to give the documents to the officer, remember to give both the original and the copy.
After this, your eligibility will be checked
Now when everything is found correct, your application is made.
