If you also want to know whether you are eligible for the scheme or not, then first visit the official portal of the scheme pmjay.gov.in
After going to the portal, click on the option 'Am I Eligible'
After this fill in the OTP received on the registered mobile number.

Then two options will appear, in the first select your state.
Now search your ration card and registered mobile number in the second option.
After this, you will know whether you are eligible or not, if eligible then you can go to your nearest public service center and apply.
