In this application, you will have to tell about the date, and time of withdrawal of money, how much money you have withdrawn, from which ATM you have withdrawn it, etc. Apart from this, you also have to attach a copy of the slip from the ATM with the application.

If you don't have the slip. In such a case, you will have to provide a copy of the transaction details. After giving all this information, the bank will immediately replace your mutilated and old notes.

Let us tell you that RBI keeps issuing circulars from time to time on mutilated and old notes. According to Reserve Bank of India rules, you can exchange only 20 notes at a time.
