Step 1
If you are eligible for Ayushman Yojana, then you can apply for Ayushman Card
To get an Ayushman card, first, you have to go to your nearest public service center.
Step 2
You have to go to the center and meet the concerned officer and tell them to apply
After that, they take some documents from you like- Aadhaar Card, Ration Card, Residence Certificate, etc.
Apart from these documents, you also have to fill in an active mobile number in the form.
Step 3
Now the documents given by you will be checked and at the same time, your eligibility is also checked.
After this, when everything is found to be fine, then the application for your Ayushman card is made.
Then within about 15 days, your Ayushman card comes ready.
