You can check whether you are eligible or not:-
Step 1
If you want to get your Ayushman card made, then check eligibility first.
For this, you have to go to the official website of the scheme pmjay.gov.in.
Here you have to click on the option 'Am I Eligible'
Step 2
Then you have to enter your 10-digit mobile number here
An OTP i.e. One Time Password will come on this filled number.
Now enter the OTP that has come here
Step 3
After this, you will see two options in front of you, in the first select your state.
Then search by entering your ration card number and mobile number in the second box.
Now you will know about your eligibility.
How to apply:-
If you are eligible for the scheme, then you have to go to your nearest Jan Seva Kendra to apply.
By going here you have to meet the concerned officer and submit your documents.
Then both eligibility and documents will be checked and if found correct, your application will be submitted.
