After this fill both these forms
After filling in the information like name, address, and account number, you will have to fill in the reason for closing the bank account.
You will also have to provide information about your other bank accounts.

Step 3
After filling in all the information the account holder has to sign the form.
If there is a joint account, then both the account holders should sign and also go to the bank.
Be sure to take your account passbook with you.

Step 4
If you do not have a passbook, then definitely carry a debit card or checkbook.
