This is the offline method:-
If you want to get an Ayushman card made, then you can go to your nearest CSC center
Meet the concerned officer here and give them the relevant documents
These documents are verified and your eligibility is also checked
If everything is found correct, your application is done.

Step 1
If you are also eligible and want to get an Ayushman card made, then for this you have to go to the official website https://beneficiary.nha.gov.in/
Here you have to click on Beneficiary fill in your mobile number and click on Verify
Now login by filling in the OTP that has come on your mobile number and the captcha code given on the screen
Step 2
After this, you have to select PMJAY in the scheme and also select your state
Then go to Sub Scheme and select PMJAY and after that select the district
Now you have to go to the search bar select your Aadhaar number and fill in your 12 digit Aadhaar number here
Then a list will open in front of you where you have to select your name and proceed to the verification
Step 3
After this, you have to upload your photo and fill in other information like OTP, and PIN code here
Then click on Submit and after 24 hours you can log in and check that Has your Ayushman card has been made or not
